​What an LPA is

A Lasting Power of Attorney lets you (the Donor), if you are 21 or older and have mental capacity, appoint one or more people you trust (Donees) to decide on:

• personal welfare (including healthcare), and/or

• property and affairs

if you lose mental capacity later.

An LPA is not a Will. A Will deals with what happens after death. An LPA is for decisions while you are alive but unable to decide for yourself.

What we do 

We certify.
Our doctors act as LPA Certificate Issuers. In person, the doctor checks that you understand the LPA, that you are making it of your own will, and that there is no obvious reason it cannot be made. If suitable, the doctor certifies and submits it on OPGO.

How to do this 

1. Draft your LPA on OPGO
Log in with Singpass at opg-eservice.msf.gov.sg. Most people use Form 1 (general powers). Form 2 (custom powers) must be drafted by a Singapore lawyer — we do not draft Form 2.

2. Your Donee(s) accept
Each Donee logs in to OPGO with their own Singpass and accepts. Do this before the clinic visit.

3. Book us for certification — $75 nett
Call the branch. Tell us it is for LPA certification so we set an appointment.

After the doctor certifies, the application goes to OPG. There is usually a waiting / objection period before it is registered. Track status on OPGO.

What to bring

​

• NRIC or other photo ID

• The same mobile phone named in your LPA, with the Singpass app installed and working (needed for digital signing)

• Your OPGO application already drafted, with Donees accepted
   

Who must come: only the Donor. Donees do not need to attend.

If the doctor is not satisfied you understand the LPA, or suspects pressure or a capacity issue, certification will not be done that day. A psychiatrist or lawyer may be more appropriate. The $75 fee is for the certification visit; we will be clear if we cannot certify.

Fees

Certification fee to the clinic will be $75 nett per certification.
